New law AB 1346, which went into effect January 1, 2012, requires that anyone who is applying to take the Electrical Certification exam attach proof of On-the-Job experience with a Social Security Administration Employment History Report.
DAS will require the first report option on the form ssa-7050 (Detailed Earnings Information). It is our understanding that individuals can take the ssa-7050 form, with the "Detailed Earnings Information" box checked, to their local office and get a print out of their Employment History with detailed earnings information which includes periods of employment or self-employment and the names and addresses of employers, however we have received conflicting information from different individuals at the Social Security Administration so we can not make any guarantee as to the availability of this report at any give Social Security Office. The link to the form is here (http://www.socialsecurity.gov/online/ssa-7050.pdf) and there is a fee depending on how many years job history is requested. SSA does not accept Credit Card or ATM; they prefer checks but will also accept cash.
Individuals can locate the SSA Office closest to them using this link, (https://secure.ssa.gov/apps6z/FOLO/fo001.jsp). It appears most offices are open M-F 9AM - 3:30PM.
Applicants with privacy concerns may redact their social security number from the report prior to submitting it to the DAS.
DAS has updated their web site with the new Certification Exam Application which now includes this requirement, and they have also provided a link to a PDF that includes instructions that applicants can use to request their SSA Employment History Report. Important note: according to the Social Security Administration this report may take up to 4 months to receive if requested by mail. Plan accordingly when scheduling an exam.
